We're inviting individuals from all over the United States to participate in a new research project and share their health and wellbeing experiences.

Participation involves a 1-hour minutes online interview where you can discuss your experiences. As a token of our appreciation, you'll receive $100 for your time and input.

When completing the form please answer carefully and as honestly as possible, there are no right or wrong answers, all answers will be looked at individually when checking that you are applicable for the research.
